Apply to the CRS Program
To apply to the CRS program, contact a CRS Case Manager:
- For any consumer or family whose language preference is Spanish, contact Amelia Paramo at (512) 407-1354.
- For consumers whose last name begins with letters A-L, contact Amelia Paramo at (512) 407-1354.
- For consumers whose last name begins with letters M-Z, contact Brandy Holt at (512) 407-1339.
You will need to provide information about the consumer with a traumatic brain injury or spinal cord injury who is a candidate for the CRS program. Once the case manager completes the consumer’s application, eligibility will be determined and the process of developing a plan of services will begin. The consumer and family will be assisted in the application process and services will be provided as soon as possible.
